CAREAPP Learning Network

The purpose of the CAREAPP Learning Network is for CAREAPP Network Partners to:

  1. Collaborate to strategize, exchange, and plan for increased engagement of CAREAPP at their respective organizations. 

  2. Connect with users from other organizations to build relationships and share best practices.

June 28, 2019

Six network partners were represented at the CAREAPP Learning Network session.

In May 2019, 24 CAREAPP network partners reached 25% user engagement, compared to 20 partners in April. The Healthify team reviewed key changes to the operational and user census reports, in addition to providing a demo of the new client merge feature.

Five network partners were represented at the CAREAPP Learning Network session. In April 2019, 225 referrals were created, compared to 173 in March, a 30% increase.  HC Department of Corrections-Reentry Program and HC Autism Society joined the CAREAPP Network.

April 26, 2019

Eight network partners were represented at the CAREAPP Learning Network session. In March 2019, 32.5% of activated users were engaged on the CAREAPP platform, a 60% increase from January.  The updated CAREAPP Service Opportunity Screening is now live and ready to use.  Handouts from Howard County Office of Workforce Development are located here.

Six network partners were represented at the CAREAPP Learning Network session. Session highlights included engagement strategies, updates to the CAREAPP screening tool, and Howard County Health Department's latest engagement strategy called Battle of the Bureaus. Seven network partners were represented at the CAREAPP Learning Network session. In January 2019, 82 referrals (Track and Coordinate) were made.  CAREAPP evaluators from University of Maryland-Baltimore conducted a focus group with participants. Handouts from Community Action Council are located here

December 12, 2018

Ten network partners were represented at the CAREAPP Learning Network session.  The project team presented the CAREAPP Navigator model to implement in the waiting room as a strategy to engage with clients and offer resource linkages.  From July 1, 2018 to November 30, 2018,  670 referrals (track and coordinate) have been created in CAREAPP.

At the first CAREAPP Learning Network session, the project team presented a snapshot of Year 1 user engagement data, accomplishments and conclusions and also reported on Year 2 user engagement progress to date.
